The Faculty Evaluation Process webpage contains a link to a single comprehensive document and in addition to links to single documents. these documents include documents for your annual evaluations. For example, the preliminary evaluation agreement document, classroom observation form, and conference forms. There is also a page for the promotion in rank process. This webpage included the application forms and the committee recommendation forms.
